The three nested holarchies — Issues, Opportunities, Goals
The chain from a felt problem to a plannable goal: what is wrong, held as one nested web; its positive mirror, what is therefore possible; and the goals that follow — all three cut by the same twelve Pillars so they can be read against each other.
Problems are rarely standalone. A shortage of food in one town, a school that no longer teaches, a river dying upstream, a people who no longer trust anyone — treat them as a list and you get a list of campaigns that undercut one another. Treat them as a nested web, where each thing both causes and is caused, and it becomes possible to intervene where the leverage actually is (Holarchy, Root Causes, System).
The first holarchy is of issues, in seven tiers beneath one meta issue. At the pinnacle: disconnection from the Divine and the illusion of separateness — the claim that every other trouble can be traced back to a broken relationship with the Creator, one another, and all creation. Beneath it: spiritual and philosophical roots · fundamental human challenges of survival, health, and identity · personal and interpersonal wounds · localized symptoms in a community or region · systemic manifestations codified into institutions and law · societal dysfunction at the scale of nations · and ecological disintegration, where humanity's separation from the living system that sustains it becomes irreversible. The chapter that carries it insists on the caveat that keeps the instrument honest: though it is drawn as a hierarchy it works as a dynamic web, and any tier may cause or be caused by any other.
The second is the positive parallel — the same seven tiers read as opportunities, beneath one meta opportunity, which is the reconnection: the one thing that makes all other things possible. The third is the holarchy of goals, whose meta goal is the Divine Symphony — right relationship restored between Creator, humanity, and all creation — and whose lower tiers translate downward, through the Pillars, into specific and interlinked commitments that a body can actually take on (Commitment, Work Breakdown Structure).
Read together they are one instrument, not three: here is what is wrong · here is what is therefore possible · here is what we will do about it. That chain is what lets a Cell take a real trouble in its own street and find both the depth beneath it and a goal it can finish in a season (Quest, Issue).

Open: The issues chapter carries ten of its own refinement considerations — among them that the tiers need a notation for their feedback loops, that the twelve Pillars are presumed static and may not be, and that no problem-solving method is yet attached to any tier.
